Pledge for Student Chromebook Use & Insurance Form

 

  • I will use and care for my District Provided Chromebook responsibly as described in the Acceptable Use Policy while at school and at home.

  • I understand that the Chromebook is subject to inspection at any time without notice and remains the property of the Millersburg Area School District.

  • I will never leave the Chromebook unsecured or unattended, and I am responsible for knowing the location of the Chromebook at all times.

  • I will not use stickers, markers, etc., on the Chromebook or do anything to permanently alter the Chromebook.

  • I will password protect the Chromebook and never loan out the Chromebook or give my password to other individuals.

  • I will not let anyone else use the Chromebook other than my teachers, administrators, or parents/guardians.

  • I will charge Chromebook’s battery daily and arrive at school with the Chromebook charged.

  • I will keep food and beverages away from the Chromebook since they may cause damage to the Chromebook.

  • I will clean the Chromebook’s screen with a soft, anti-static cloth only, using no cleaners.

  • I will not disassemble any part of the Chromebook or attempt any repairs.

  • I will not remove or deface the serial number or other identification on any District Chromebook.

  • I will cooperate in the filing of a police report in case of theft, vandalism, or other acts covered by

    insurance and inform my building administrators.

  • I will be responsible for all damage to, or loss of the Chromebook, caused by neglect or abuse.

  • I agree to return the Chromebook and power cords in good working condition.

 Chromebook Insurance: $25.00 - Checks payable to Millersburg Area School District

*This is a non-refundable fee and only applies to the 2022-2023 school year.

 Opt Out - I have reviewed the stated Pledge for Chromebook Use and Acceptable Use Policy and choose to opt OUT of the District’s insurance. Since insurance is not being purchased, I understand that I will be financially liable for the full replacement or repair cost as determined by the Technology Department and building principal.

Insurance Coverage Terms

The Millersburg Area School District Student Chromebook Insurance includes, but is not limited to, covering the following instances.

  • Liquid spilled on or in the Chromebook

  • Drops, falls, and other collisions

  • Electrical surges

  • Damaged or broken LCD due to a drop or fall

The MASD Student Chromebook Insurance will cover TWO (2) repairs, or ONE (1) Chromebook replacement, per School Year beginning on August 1 and ending on July 31.

Any additional repairs will be the responsibility of the student’s Parent/Guardian as determined by the Millersburg Area School District Administration.

The MASD Insurance does not cover, but is not limited to, the following instances.

  • Damage due to fire

  • Intentional damage (such as hammer marks)

  • Cosmetic damage (unless such damage affects the safe-use or functionality of the Chromebook)

  • Theft
